 Please arrive promptly for your appointment. If you are a new patient, plan to arrive and
check in 20 minutes prior to your appointment time. This allows sufficient time to gather or confirm the medical and financial information we need for your records. If time allows, we will mail a medical questionnaire and information to you prior to your appointment. Please bring this completed form with you to your visit.
If you have been a patient in our office before, please plan to check in five to 10 minutes before your appointment time. This gives us time to verify that the information we have in our files is correct and current. |
